Position Description

Serve on Regional Care Teams at the direction of the CBPNs, Complex Case Managers and Behavioral Health Specialists.

Develop relationships with high volume Emergency Departments and hospitals.

Provide support for frequent Emergency Department users to identify causes and implement client education about use of Medical Home and other ways to utilize appropriate care services.

Focus on high risk, complex care coordination needs, especially for clients with behavioral health and complex case management needs; coordinate social services needs with local organizations.

Provide expertise in coordination and assistance with processing applications to clients who qualify for alternative benefit options.

Assist in improving self-care and self-advocacy.

Minimum Requirements

Position is located in remote Texas territory, Valley area.

Education/Training

-Social Worker (BS, or MSW) from an accredited school

-3+ years experience in clinical social work

-Current unrestricted license in the state that they reside.

-Certified Social Worker in Health Care (C-SWHC) preferred.

-Current member of the National Association of Social Workers preferred

Specialized Knowledge/Skills:

-Windows environment (preferably Word and Excel)

-Excellent written and verbal communication skills.

-Excellent organizational skills.

-Current PPD.

-Current valid driver¿s license and required level of automobile liability insurance coverage.

-Thorough knowledge of social work practice and the application to case management including complex case management and behavioral health.

-Understanding of family and group dynamics.

-Familiarity with change behavior techniques.

-Knowledge of government and private organizations, resources in the community, laws, regulations and policies that govern clinical social work.

-Skill in establishing rapport with members, families, and local resources (medical, social, civic, legal, religious organizations)

-Ability to communicate and execute work effectively.

-Preferably a member of the local community.

Working Conditions:

- Environment ¿ Independent, home environment with daytime travel within an assigned specified catchment area. Must have access to high-speed internet connection (e.g. DSL, Cable) and be able to comply with Work at Home Agent Requirements.

- Travel: Willingness and ability to travel locally, reliable mode of transportation required.

Current Need

Full-time Case Manager in Kern County, CA.

Position Description

The goal of the Case Manager is to facilitate the delivery of optimal healthcare while promoting fiscal responsibility. Participates in the coordination and access to care for members with high cost/high risk conditions. The Case Manager will address member needs after the completion of a health assessment and develop appropriate member focused interventions. In addition, the CM evaluates available resources, plans, and implements necessary services in order to monitor outcomes of identified problems. As an integral member of the Care Team, the CM will collaborate with other members of the health care team to coordinate an integrated approach to member management. This role may be hospital-based, community-based, long-term facility-based or telephonic.

Location / Work Environment

-Kern County, CA

-This is a telephonic and field based position.

-Environment: Independent, home environment with daytime travel to assigned demographic operational area and facilities. Must have access to high-speed internet connection and be able to comply with Work at Home Agent Requirements.

Minimum Job Qualifications

- Registered Nurse with active, unrestricted CA nursing license.

- CCM certified or eligible within two years.

- Five+ year¿s clinical experience, preferably acute care.

- 3+ years case management experience.

- Strong computer proficiency.

-Bi-lingual in Spanish strongly preferred.

- Current CPR certification.

- Current PPD.

- Current valid driver¿s license and required level of automobile liability insurance coverage.

Education

Registered Nurse with active, unrestricted CA nursing license.
